East Tennessee Psychological Testing (ETPT) is now closed as Dr. Axtell has moved out of state. I no longer have a fax number associated with ETPT. Please use this contact page only if I have seen you in the past and you need a copy of your report. If you have an outstanding balance with me your balance will need to be paid in full before I will release the report; as indicated in our payment agreement. You can pay your balance by calling my office number listed on this page. I will not reply to any voicemail messages except to take payments.
